Wallace Figure Skating Club forms

The WFSC would like to help its members stay informed and have all the forms they need to advance their career in Figure Skating. If there are any other information you wish were on this site please email wfsc@WallaceFSC.com with your ideas. Suggestions and comments are always welcome!

*Each Basic Skills or higher skater must have a current WFSC/USFS membership. Please see the Membership Form for details.

*Any skater on WFSC ice must have a completed Waiver and Medical Attention Form on file.

*WFSC would like to recognize our members' accomplishments. Please turn in a Publicity Permission Form to let us know if we may publish information about your skater.

*Registration Option Forms allow you to choose which options (ice times) you would like to register for. Basic Skills, Freestyle and Power Edge Group classes are available as well as Open Ice Sessions for skaters wishing to take lessons with a private Professional Coach.

*Learn To Skate Registration Forms allow you to choose which group session you would like to register for.

*The WFSC will be hosting a U.S. Figure Skating Test Session for Moves In the Field and FreeStyle, on Monday, February 20, 2012, 8:00 am - Noon. MIF & FS Test Form and Dance Test Forms are due by February 6, 2012.

*WFSC Gear will keep you and your skater warm and comfortable this season? Complete the Gear Order Form and turn into the clubroom.

*WFSC is a nonprofit organization made up entirely by volunteers. Our professional coaches are the only individuals receiving compensation, but they also volunteer a great deal of their time to help organize our events. Throughout the season, many volunteers are also needed for all kinds of jobs, including committee help. Check out the Volunteer List to see if there is a task or two you could help out with.
